Balancing underdiagnosis and overdiagnosis: the case of mild traumatic brain injury.

چکیده
M ild traumatic brain injury (m-TBI) is a public health problem, particularly in veterans and athletes. Often synonymous with ‘‘concussion,’’ m-TBI is head injury accompanied by acute-phase characteristics, such as alteration of consciousness. m-TBI can lead to chronic neuropsychological symptoms, known as postconcussive syndrome (PCS), and has been linked to chronic traumatic encephalopathy, a progressive neurodegenerative disorder. Managing patients with m-TBI is challenging because of the difficulty in predicting outcomes. Furthermore, it is challenging to distinguish between the effects of m-TBI and the psychiatric conditions which frequently coexist. Advanced neuroimaging can identify structural brain damage related to m-TBI which is not detectable with conventional brain magnetic resonance imaging. The most promising techniques are diffusion-based, such as diffusion tensor imaging (DTI), which detects changes in the diffusion properties of the white matter that reflect microstructural injury. There are significant differences in scalar diffusion metrics in patients with m-TBI compared with controls (1), and these differences correlate with axonal damage in animal models of m-TBI (2). Diffusion measurements correlate with symptoms, objective measures of neurocognitive function, and outcomes. The research, thus far, induces hope that diffusion imaging techniques will identify individuals with structural brain injury and complement clinical decision making. Although the need for further standardization and refinement of these techniques before clinical implementation in m-TBI is recognized (3), it is timely to speculate on their potential impact in the real world.

Effect of Mild Brain Traumatic Injury on Intelligence and memory Function in Motorcycle Riders
Introduction: The most common causes of traumatic brain injury are vehicle crashes, including motorcycles, which lead to long-term disabilities. The purpose of this study was to investigate the effect of mild brain trauma on intelligence and memory function in motorcycle riders suffering from mild tumor injury. Materials & Methods: In this prospective cohort study, intelligence and memory fu...
متن کاملPrediction of Clinically Important Traumatic Brain Injury in Pediatric Minor Head Trauma; proposing Pediatric Traumatic Brain Injury (PTBI) Prognostic Rule
Background: The present study assesses independent predictors of clinically important traumatic brain injury (ciTBI) in order to design a prognostic rule for identification of high risk children with mild head injury. Materials and Methods: In a retrospective cross-sectional study, 3,199 children with mild traumatic brain injury (TBI) brought to emergency ward of three hospitals in Tehran, Iran...
متن کاملPrediction of mental disorders after Mild Traumatic Brain Injury: principle component Approach
Introduction: In Processes Modeling, when there is relatively a high correlation between covariates, multicollinearity is created, and it leads to reduction in model's efficiency. In this study, by using principle component analysis, modification of the effect of multicolinearity in Artificial Neural Network (ANN) and Logistic Regression (LR) has been studied. Also, the effect of multicolineari...
متن کاملComparison of visual perception error among patients with mild traumatic brain injury with healthy people
Background: Mild traumatic brain injury (MTBI) is the least brain injury due to direct or indirect blow to the head, which according to the Glasgow scale is given a score between 13 and 15. The findings indicate cognitive impairment. Perceptual dysfunction is one of the most important cognitive deficits in these patients. The present study was designed and performed to compare MTBI patients wit...
